IMPORTANT INFORMATION ABOUT THE NY HEALTH EXCHANGE:
We will be part of the Oscar network until December 31, 2016. As of January 1, 2017, Concorde physicians are only eligible to participate in BC/BS Pathways on the NYS Health Exchange.
We at Concorde are very concerned about preserving the continuity of your care when these changes take place. Please check with your physician’s office to find out what other health plans your doctor accepts. You may be able to stay with Concorde for your ongoing care if you choose a different plan on the Exchange during “Open Season” which starts in November.
Oscar's 2017 directory of participating providers will be available September 30, 2016. At that time, you may check the directory at hioscar.com or by calling Oscar at 1-855-OSCAR-55. You may also check with your physician's office to obtain more information about our plan participation.
